What do Consultative Nurses Do?
Consultative Registered Nurse supports individuals with intellectual and developmental disabilities (IDD) who live in the community setting. Responsibilities include health promotion and advocacy, teaching people with IDD and those who support them about health conditions and treatments and overseeing a person’s general health care.
Care management of auditing monthly documentation and communicating with team members helps maintain the individual’s health, independence, and inclusion in their community.
Registered Nurse (RN) Consultative Services include:
Act as the primary health care coordinator and develops the nursing plan
Participate as an Interdisciplinary Team member
Attend the annual ELP Meeting
Provide ongoing health related training for staff of individuals on their case load
Attend medical appointments as needed
Maintain ongoing health related training for staff of individuals on their case load
Attend medical appointments as needed
Maintain ongoing documentation for health care status
Communicates to individuals/families/guardians/other service providers about health care issues
Coordinate with hospital for care as needed, including admissions and discharges
Assist in obtaining resources and act as coordinator of health care services
Complete record reviews (e.g. the monthly medication review) per the appropriate schedule
Assist with individual's transitions for one site to another
Act as an advocate for individuals on their caseloads
Complete the annual ELP nursing assessments and other assessments as appropriate
